________'s Phone Contact Log: A Free Template for Legal Safety

I didn't understand until I was in my first year of teaching SPED how important keeping data was--not on students, but on myself.

SPED is a really legal heavy arena of education, and even if you are a general education teacher, it's really important that you start to keep a log of information about interventions and other times you've done particular actions.

Here, I wanted to share with you a Google Form that can help you keep a log of phone calls home.
Because there's no written transcript that's saved when a phone call is made, it's particularly important that you take the time (even if it's just two minutes) to document who was called, what was discussed, etc.

Here's what mine looks like. Notice that I tried as much as possible for it to be quick and concise but still leave options for a variety of responses.
